Elevate+ for Public Health.
Solving the last mile.
California has spent years building something extraordinarily valuable: a statewide infrastructure that helps millions of Californians understand what healthier living looks like. The next opportunity is helping people put that knowledge into practice — every day.
Reach isn't the problem.
Persistence is.
CDPH's work targets the communities most vulnerable to obesity, high blood pressure, and type 2 diabetes — combining education, social marketing, and community-level interventions at a scale no other implementing agency matched.
A poster can reach someone. A class can educate someone. A healthier school environment can improve choices. But none of them provides an individualized feedback loop, every single day.
Converting reach into persistent individual behavior — that's the role Elevate+ is built for.
Your mission.
Made daily.
These aren't our words — they're yours. Every commitment your program made, Elevate+ helps keep.
"Improve the health of families and communities most vulnerable to obesity, high blood pressure, and type 2 diabetes."
Elevate+ is designed to prevent. It understands each participant's health risks and quietly steers every meal, every suggestion, and every daily habit away from obesity, type 2 diabetes, and high blood pressure — before they ever take hold. Not treatment. Prevention, delivered one meal at a time.
"Combine education with social marketing and community-level change."
The individual layer your model has been missing: a daily check-in that starts from how the participant feels, a "Today's Focus" built from their actual data, and weekly insights that spot the patterns behind the behavior — late-night eating, liquid calories, skipped breakfasts, low fiber. And in-app Communities let a class, clinic, or neighborhood cohort keep encouraging each other long after the program ends.
"Healthy Victories — small, achievable changes that add up to larger long-term health victories."
Healthy Victories, operationalized as a five-week behavior arc — Build Awareness, Find Consistency, Gentle Refinement, Momentum. One improved meal, one smart swap, one glass of water at a time, with every small win visible on the participant's own dashboard and home-screen widget.
"Sustain the impact built across 61 local health departments — beyond SNAP-Ed funding."
Scale without staffing: a digital continuation layer any jurisdiction can deploy, where local staff pair with participants through Coach Mode. And we envision building aggregate outcome dashboards with your evaluators — evidence for the next funding conversation.
What this looks like in practice.
Small changes.
Healthy victories.
CDPH's Healthy Victories approach says it best: achievable, incremental improvements produce larger long-term health victories. Elevate+ is that philosophy, operationalized — one meal, one choice, one day at a time.
Instead of another library of health information, participants get answers to the practical questions that arise throughout the day: What should I eat next? How am I doing today? How can I improve this meal? What small change can I make right now?
Guidance that adapts
to each person.
Participants log meals through simple text, voice, barcode, or photo. Elevate+ translates broad nutrition guidance into personalized, actionable recommendations based on each person's goals, preferences, food traditions, and eating patterns.
For local health departments, it becomes the digital continuation layer between interventions — maintaining engagement with participants between in-person touchpoints while generating measurable indicators of participation and behavior change.
Complement.
Never replace.
Local health partners remain the trusted human connection. Elevate+ can provide ongoing reinforcement between those interactions.
Define the Cohort
This could begin with a single population or local health jurisdiction — whatever scope makes sense for your teams.
Deploy Alongside Programs
Elevate+ reinforces the guidance trusted educators already deliver — it doesn't compete with it.
Measure Everything
Engagement, dietary behavior, physical activity, and sustained participation — evaluated before any conversation about broader deployment.
Scale What Works
A validated model that lets substantially more Californians receive personalized support — without a proportional increase in program staffing.
"You've already solved awareness at enormous scale. We want to help solve the last mile — between knowing what to do and actually doing it at 7:30 PM, when someone is deciding what to eat."
