Elevate+ for Community.
Elevate+ is a personalized wellness platform powered by Nova — an AI nutrition and fitness coach that meets every participant exactly where they are: their food culture, their budget, their health.
What programs like CalFresh Healthy Living teach, Nova helps people live — one meal, one habit, one day at a time. Always there, right in their pocket — even at 7 PM, when dinner needs deciding and the class ended days ago. So communities don't just learn how to prevent diabetes, obesity, and heart disease — they live it.
CalFresh Healthy Living made the healthy choice the easy choice.
Elevate+ makes it the easy choice at every single meal.
Decades of impact.
Worth preserving.
From 2018 to 2024, CalFresh Healthy Living reached more than 18 million Californians through nutrition education, physical activity programs, and community partnerships — delivered by the Department of Public Health, the University of California, Catholic Charities, and the Department of Aging. That work is prevention at its best — reaching families before obesity, type 2 diabetes, and high blood pressure take hold.
With the elimination of federal SNAP-Ed funding in 2025, those agencies now face a defining question: how do you preserve the knowledge, relationships, and momentum built over decades — with a fraction of the staffing and budget? And the demand is documented: peer-reviewed research has consistently found that the overwhelming majority of California SNAP participants support strengthening the program's nutritional impact. The people these programs serve are asking for more support, not less.
Elevate+ can help. Not by replacing community programs — but as a sustainability layer that lets their impact continue between and beyond every in-person touchpoint. And it's health equity in practice: personalized daily nutrition support — long a privilege of those who can afford dietitians — delivered to the communities that need it most.
Not another nutrition app.
A population-health platform.
Condition-Aware Targets
Not vague advice — real numbers. High blood pressure lowers a participant's sodium target to 1,500 mg. Prediabetes and diabetes tighten sugar targets — catching risk in the prevention window, before it becomes disease. Obesity and diabetes prevention, built into every recommendation.
Culturally Adaptive
Onboarding captures each person's food background — cultural and traditional dishes, family meals, everyday staples — and every recommendation is required to fit it. Guidance that speaks each family's own food language.
Guideline-Grounded
A built-in Health Sources screen cites the Dietary Guidelines for Americans, CDC, AHA, WHO, NIH/NIDDK, and ADA by topic — and a "Sources" chip appears right under substantive guidance. Participants can always see where advice comes from.
Zero-Friction Access
Speak it, snap it, scan it, or type it — including nutrition-label scanning — then fix anything with one tap. No nutrition literacy or complicated data entry required.
Communities Inside the App
Participants don't go it alone. Communities inside the app let a class cohort, a pantry's families, or a senior center's regulars encourage each other between program days — because connection is half of behavior change.
Safety by Design
Hard calorie floors, safe pace limits, and clamped targets are enforced in code — and when patterns look medically risky, Nova keeps advice conservative and suggests checking with a clinician. It never diagnoses. Data lives on the participant's device, no account required.
And where we want to take it — together.
Capabilities we envision building alongside California's community health agencies — shaped by the people who know these programs best.
Budget-Aware Guidance
"Dinner options under $4 per person." Cost-aware recommendations that stretch limited food dollars — SNAP-Ed's oldest promise, made personal.
Program Dashboard
Aggregate outcomes, not individual surveillance: enrollment, 7/30/90-day retention, engagement, and pre/post indicators by cohort — designed with your evaluators.
Multilingual Experience
The cultural intelligence is already built in. A fully translated experience — prioritized by the languages your communities actually speak — comes next, with your partners guiding the order.
From episodic education
to continuous support.
Community programs remain the trusted human relationship. Elevate+ provides the reinforcement in between.
Education
Trusted educators teach evidence-based nutrition and physical activity — in classrooms, pantries, senior centers, and clinics.
Daily Application
Elevate+ translates those lessons into personal, practical answers: what to eat next, how to improve this meal, one healthy step today.
Sustained Change
Small, achievable improvements compound. Participants stay engaged between sessions instead of starting over after each one.
Measurable Outcomes
Programs see engagement, retention, and behavior indicators — evidence of impact that outlives any single funding cycle.
Your promises.
Kept daily.
CalFresh Healthy Living made California a set of promises. Here's how Elevate+ helps keep every one of them.
"A California where everyone is healthy, active, and nourished."
A personal coach for every Californian, not just program attendees — one that knows their goals, conditions, and food culture, and shows up every single day.
"Teach Californians about good nutrition."
Education that doesn't fade after the lesson: guideline-grounded guidance with visible sources — Dietary Guidelines, CDC, AHA, WHO — applied to the actual meal in front of them.
"Help families stretch their food dollars."
Meal plans built from the foods families already have and love — with repeatable, low-effort options. Explicit cost-aware recommendations are next on our roadmap, co-designed with agency partners.
"Make the healthy choice, the easy choice."
Zero-friction by design: speak a meal, snap a photo, get one achievable improvement. Condition-aware targets make the right choice the default, not a research project.
"Build partnerships in communities."
In-app Communities keep a cohort encouraging each other between program days, and your educators stay in the loop — pairing with participants, seeing progress, and leaving guidance the AI follows. Technology that strengthens the human relationship — never replaces it.
What this looks like in practice.
The class ends Tuesday.
Dinner is Thursday.
A poster can reach someone. A class can educate someone. A healthier school environment can improve choices. But none of them is there at 7:30 PM, when a person stands in their kitchen wondering: "Okay… what should I actually eat tonight?"
Elevate+ answers that question — based on what they normally eat, their food traditions, their budget, their goals, and the guidance their program already gave them. That's the last mile between knowing what to do and actually doing it.
Their food. Their culture.
Their plan.
Elevate+ doesn't hand every family the same salmon-and-quinoa meal plan. Onboarding captures each person's food background — cultural and traditional dishes, family staples, everyday favorites — alongside their age, dietary preferences, and medical conditions. Every recommendation is required to fit all of it.
Keep your dal — here's the portion and the pairing. Calorie and protein targets calibrated to their body and age. Sugar and sodium automatically adjusted for diabetes or blood pressure. And because there's no fixed food database, any cuisine works — the guidance meets each person exactly where they are. Culturally responsive, equity-centered care — not one-size-fits-all advice.
Not a tracker.
A habit builder.
Real change is built from small daily reps. A check-in that starts with how the person feels. A Today's Focus built from their actual data. One achievable improvement per meal. Weekly insights that reveal the pattern behind the habit — and exactly what to adjust.
A five-week arc — Build Awareness, Find Consistency, Gentle Refinement, Momentum — turns showing up into a routine. Even the reminders are written to encourage, never to guilt. That's how education becomes behavior, and behavior becomes health.
Impact you can
demonstrate.
Community programs have always measured behavior change through pre/post instruments. Elevate+ adds what surveys can't capture: what participants actually do, every day, between measurements.
Daily engagement, dietary patterns, and behavior — captured continuously with a full history of every change. We envision working with your evaluators to turn that into cohort-level evidence — monitoring, evaluation, and reporting designed with the people who will actually use it.
One vision.
Four missions.
Each implementing agency built something different. Each deserves its own story — not four versions of the same email.

Awareness at enormous scale is solved. The last mile — turning reach into persistent individual behavior — is the next frontier.

California built.
We'd welcome the chance to start small — one population, one jurisdiction — and measure everything before talking about scale. And if you lead, fund, evaluate, or fiscally sponsor community health programs, we'd love to talk.
